Lake Oswego School District is partnering with Oregon Health & Science University (OHSU) to implement the K12 COVID-19 Student Screening Program for your child to get free weekly COVID-19 PCR tests through your school. The program is funded by Oregon Health Authority and implemented by designated providers.
The testing program is voluntary and requires active permission from students’ guardians. We are currently enrolling LOSD elementary students. The enrollment deadline is Friday, Sept. 24 at 3:30 p.m. Following is more information and instructions on how to enroll your student today. To enroll, following are links to the Student Packet #1, which includes 3 documents:
- The introductory handout providing an overview about why guardians should consider enrolling your student in the program
- The OHA consent form to print, complete, and submit to your school by September 24. **
- The OHSU authorization form to complete, print and submit to your school by September 24.
** In the OHA consent form, parents-guardians need to select the third option if you choose to enroll in this K12 COVID-19 Student Screening Program. If you do not, then the form is not valid. (The first option is for the BinaxNow diagnostic testing in school for students and staff who become ill with COVID-19 symptoms while at school; the second option is not recommended by Clackamas County at this time.)
The OHA consent form and the OHSU authorization form need to be completed, signed and delivered to your school office by Friday, Sept. 24 at 3:30 p.m. You may give the forms to your student to turn in at the start of class, and teachers will make sure the forms are submitted to the office. Or, guardians may drop off forms directly to the school office during school hours. Printed copies of these forms are being sent home in your child’s backpack today, and extra printed forms are available in your school’s secure vestibule.
